Members living with ongoing health conditions may be eligible for additional assistance from a nurse, case manager or social worker.
Our care managers work with eligible members one-on-one to help them achieve their best health while living with a chronic condition. The care manager will offer support by listening to your needs and checking in regularly by phone, video or home visit.
Eligible members living with multiple chronic conditions may receive additional personalized support over the phone. This could include care plan development and oversight, referrals for services and help reaching health goals.
